When filling out the Online Application, please have your credit card information ready to pay the $25 initial application fee at the end of the form. FIDM accepts Visa, Mastercard or American Express.

Or, download the PDF formatted Printable Application form, open it with Adobe Acrobat Reader, then print and complete the application form. Be sure to provide the application materials and appropriate fees described in items 1 - 5 and mail to the address below (item7).

Submitting an application is not an obligation for an applicant to attend FIDM nor is FIDM obligated to accept the applicant.
